Reminder to update your current Insurance information online or in-office. 

If you have insurance, we must now have that information to be able to send any electronic QUEST Orders. Please Add, Update, and Verify your Current Insurance Information listed under your Patient Portal “My Account Details” tab. 

The insurance information to be completed:

• Your Member ID 
• Group Number (if applicable)
• Subscriber Name 
• Relationship to patient 
• Subscriber's date of birth 
• You will also need to list the Medical Claims Address and/or Phone Number

Quest can draw the bloodwork In-office or at any outside Quest location, should you reside outside of the St. Louis Metro area, where it is not feasible to travel to have your labs drawn In-Office.
