Okay to take medication that does not require food. Please hold any Thyroid medication until after the blood draw.

It is Now Required that you Update the "My Account Details" TAB Listed in Your Patient Portal. Under "My Account Details" You Can Add, Update, and Verify Your Current Health Insurance Information for QUEST Laboratory Services. Please Know That Your Health Insurance Information is Used ONLY to Place Lab Orders.

 

If you have Commercial Insurance, Medicare, Medicare plus a Medicare Supplement, Medicare Advantage, or TriCare Insurance, we must now have that information ON FILE to send any QUEST Orders. Please Add, Update, and Verify your Current Insurance Information listed under your Patient Portal “My Account Details” TAB. The insurance information to be completed includes Your Member ID and Group Number (if applicable), Subscriber Name, Relationship to you, and the Subscriber's date of Birth. You will also need to list the Medical Claims Address and/or Phone Number.

 

Once your insurance information has been added into the Patient Portal, we can electronically send your lab order to Quest. Quest can then draw the bloodwork In-Office or at any Outside Quest location, should you reside outside of the St. Louis Metro Area, where it is not feasible to travel to have your labs drawn In-Office.

 

Please Note: If CHOOSE NOT to use your health insurance for QUEST laboratory services or you Do NOT HAVE health insurance for QUEST laboratory services and would rather be Cash Pay for these services, you now have the option to be Cash Pay via Health By Design MD New Client Account Option. Here’s How to Sign-Up for this New Cash Pay Election!

1.     Go to Patient Portal Questionnaires TAB.

2.     Scroll to the bottom of the page to (NEW) Self-Pay Patient Option for Laboratory Services for Noninsured or High Insurance Deductible Plans

3.     Complete the Form Titled: PATIENT ELECTION TO SELF-PAY UPFRONT FOR LABORATORY REQUISITION SERVICESIn addition, you will need to list QUEST on the form as the laboratory that you electing to be self-pay.  

4.     Once we have received Your Signed Election to be Self-Pay you will then be charged at Health By Design MD's Client Cash Pay Rate.

5.     This is a discounted rate which is comparable to the discontinued Howell Lab. With the Health by Design MD discounted rate, we require that you acknowledge our terms and conditions by completing the PATIENT ELECTION TO SELF-PAY UPFRONT FOR LABORATORY REQUISITION SERVICES form. Please list "Quest" in the Field for Laboratory. Please Make Sure to list the "Plan Type" or "None" in the Field for Insurance.

6. Once you've signed the PATIENT ELECTION TO SELF-PAY UPFRONT FOR LABORATORY REQUISITION SERVICES, we'll provide you with Health By Design MD "QUEST" Client Pricing. *Please note, there is a $5.54 draw fee.
