Important Reminders:

Insurance Information Required for Lab Orders & Pharmacy Prior Authorizations
If you plan to use insurance, we are now required to have complete insurance details on file in order to submit Quest lab orders electronically.

Please update your information in the Patient Portal under “My Account Details.”

Be sure to include:

• Member ID
• Group Number (if applicable)
• Subscriber’s Full Name & Date of Birth
• Relationship to Patient
• Medical Claims Address and/or Phone Number

Lab Draw Options
Labs can be drawn in-office or at any Quest Diagnostics location. If you are located outside of the St. Louis area, you may choose the Quest location nearest you.  If you prefer any other labs please let us know so we can provide you with the correct order forms.
